FIT Test Requests

This FIT Test Request interface simplifies the process by guiding patients through eligibility checks and collecting essential information, reducing the burden on clinic staff and minimizing back-and-forth communication. Here’s how the system works:

1. Confirming the Patient’s Request

The first step informs patients about the two primary types of screening tests. Patients must confirm which test they are requesting.

  • Colonoscopy: If selected, patients are instructed to schedule an appointment with their physician for further discussion.

  • FIT: If selected, patients proceed to the next step to check eligibility.

2. FIT Test Eligibility Check

Patients who choose the FIT option are directed through a brief eligibility screening. They must answer a few questions to determine if they qualify to receive a FIT Test kit by mail.

3. Eligibility Check Outcome

  • Eligible Patients: Patients who qualify for the FIT Test will be prompted to provide an updated mailing address for the laboratory to send the kit. They will also be informed of the timeline for receiving the kit. Before submitting the request, patients can preview and confirm their details.

  • Ineligible Patients: Patients who are not eligible are redirected to a page advising them to schedule an appointment with their physician for further evaluation.
